IHE -- Actor, Transaction, Profile

首先，IHE = Integrating the Healthcare Enterprise = 集成医疗机构
这里，I不是名词，是动词，按照汉语对名词和动词的前后安排，“集成医疗机构”不如“医疗机构的集成”在翻译上来得更合适——“集成”在这里可以当作动名词，反译回去，Healthcare Enterprise's Integrating与原名基本吻合。

但是，由于IHE在我国的理解是一套规范——虽然不是标准——，“医疗机构的集成”作为一套规范的名称，其中的“的”似乎不大为大伙接受，所以，我也是愿意采用“集成医疗机构”这个翻译法。

For each Integration Profile:
the context is described (which real-world problem)
the actors are defined (what systems are involved)
the transactions are defined (what must they do)

Actor是泛泛意义上的执行系统
Transaction是Actor之间的互动(互操作)
Profile是具体现实问题及其解决方案的场景
(开始时我是译作“Profile是具体现实问题的场景、及解决该问题的推荐方案”，但推敲老外如此陈述的心理，实际上并没有把意思最终落在“解决方案”上，而是落在对现实场景的描述上！！！)

对于每个Integration Profile：
场景(Context)要被描述出来(是现实世界的哪种具体问题)
actor要被明确定义(要用到什么样的实施系统)
transaction要被明确定义(上述的actors要干些什么)

TRANSACTION的取意：
应为Merriam-Webster Online的第二项解释，包括2a和2b
应为American Heritage Dictionary的第一和第三项解释

结合上面Kevin O’Donnell的定义，TRANSACTION在IHE中的翻译，以大家公认的“交易”最为合适——首先有“交”，即互相碰面说上话；其次有“易”，即有交换行为和交换的东西。
实际上我碰到的多数人喜欢把TRANSACTION翻译成“事务”。这个翻译法虽然更加文雅而使大伙心理舒服，但离其本意太远。只是因为大家把“交易”往往局限在买卖上，而且不够“文”和“雅”，所以心理上有些抗拒“交易”罢了。实际上老外在心目中，使用TRANSACTION就是取其有“交”有“易”的意思，如果咱们为了“雅”而抛弃了“信”与“达”，就舍本求末了。

PROFILE的取意：
应为Merriam-Webster Online的第4项解释
应为American Heritage Dictionary的第5项解释

结合上面Kevin O’Donnell的定义，实际上PROFILE在IHE中翻译成“事务”，比把TRANSACTION译为“事务”为合适——现实场景的问题及解决该问题的推荐方案。
但是，这样会与许多人心目中的TRANSACTION混淆。

实际上，从取词倾向性上讲，我也比较赞同“听雨弹剑”所建议的“规范”。但是，“规”的原意是gauge，“范”的原意是mold，总感觉英语世界的人的心目中的PROFILE是一种描述性的、而非强制约束性的意义。
至于“规程”的“程”字，原意为procedure或process，虽然有点意思，但是离scenario感觉还是差许多。

为了贴切，也许“事图”更为确当？因为PROFILE就是来源于“用图勾勒”。但这个词又难以为大伙认可。

ACTOR——执行者(器)(直接译成“演员”也未尝不可，呵呵)——译成“执行者”还是有“说大词”的思维定势在作怪！
TRANSACTION——交易
PROFILE——事图

   2000/2001的七种Profile(Year 3)：
   1、病人信息核对校正：PIR (Patient Info. Recon-ciliation) provides an efficient method to handle the reconciliation of information for cases where procedures are performed on unidentified or mistakenly identified patients.
   2、预约流程：SWF (Scheduled Workflow) specifies the flow of information for the key steps in a typical patient imaging encounter (registration, ordering, scheduling, acquisition, distribution, and storage.
   3、显示组合影像检查：PGP (Presentation of Grouped Procedures) enables management of cases where images for multiple procedures are acquired in a single acquisition step (for example, spiral CT of the chest and abdomen).
   4、影像展示的一致性：CPI (Consistent Presentation of Images) makes it possible to ensure a consistent view of images and annotations across different displays and media.
   5、关键影像标记：KIN (Key Image Notes) allows the addition of textual notes and pointers to key images in a series.
   6、简单影像和数值报告：SINR (Simple Image & Numeric Reports) implement a standard way of creating, managing, storing, and viewing reports that including image, text, and numerical values.

   2002 新增的三种Profiles(Year 4)
   8、基本安全措施：SEC (Basic Security) establishes basic security measures that can, as part of an institution’s overall security policies and procedures in the enterprise, help protect the confidentiality of patient information. It also provides institutions with a mechanism to consolidate audit trail events on user activity across several systems interconnected in a secure manner.
   9、收费信息发送：CHG (Charge Posting) specifies the exchange of information from the Department System Scheduler/Order Filler actor to the Charge Processor actor regarding charges associated with particular procedures, as well as communication between the ADT/Patient Registration and Charge Processor actors about patient demographics, accounts, insurance, and guarantors.
   10、后处理工作流程：PWF (Post-Processing Workflow) addresses the need to schedule and track the status of the steps of the typical post-processing workflow, such as Computer-Aided Detection or Image Processing.
   2003 新增的二种Profiles(Year 5)
   11、证据文档：ED (Evidence Docs) defines interoperable ways for observations, measurements, results and other procedure details recorded in the course of carrying out a procedure step to be output by devices, such as acquisition systems and other workstations; to be stored and managed by archival systems; and to be retrieved and presented by display and reporting systems.
   12、诊断报告工作流程：RWF (Reporting Workflow) addresses the need to schedule and track the status of the reporting workflow tasks such as interpretation, transc ription and verification.
   2004新增的二种Profile

IHE IT基础架构 (ITI)
   IHE IT Infrastructure Technical Framework的2005年8月15日版(2.0版)，分为第一卷和第二卷，第一卷对每个集成事务图(Integration Profile)提供了高层次的概述，一个集成事务图具备了解决特定IT基础架构需求的特定能力。第二卷(ITI TF-2)对于集成事务图中的交易域transaction所使用的具体技术进行了详细的描述。IT基础架构技术框架2.0版包含9个Profile，Profile之间存在依赖关系：
   1、Retrieve Information for Display (RID)：显示所需信息获取。提供一种简单快捷的方式来获取必要的患者信息。此profile支持对已存储文档的读取，包括CDA、PDF、JPEG等流行的文档格式。另外，为了临床的需要，此profile还支持读取某些以病人为中心的关键信息，例如过敏信息、当前用药、报告汇总等。
   2、Enterprise User Authentication (EUA)：机构用户验证――为每个用户分配唯一的用户名，此用户名可以登录进入企业的所有的设备和应用程序。这样，可以极大的方便医院内部的用户授权、验证和管理工作。在此基础上，可以通过支持单点登录(single sign-on)方式，为用户提供很大的方便。此profile是在Kerberos (RFC 1510)标准和HL7的CCOW标准的基础上建立的。
   3、Patient Identifier Cross-referencing (PIX)：患者ID交叉索引――在多个患者ID域之间，提供同一个的患者标识的相互索引。一旦这多个系统建立了患者ID交叉索引，同一个患者即使在多个信息系统中有不同的ID，也可能同时从多个系统中获取患者相关的信息。
   4、Patient Synchronized Applications (PSA)：患者同步应用――用户可以在一台电脑上，同时使用多个独立的应用程序上浏览同一个患者的数据信息，减少了用户在多个程序中分别选择此患者的重复操作。此事务图profile是基于CCOW标准的，尤其是CCOW中关于“患者”主题的上下文管理部分的内容。
   5、Consistent Time (CT)：一致时间――这是一套在多个系统和多台电脑之间保证时间一致的体系结构。IHE中很多其他profile都要求多台电脑间保持时间的一致。此profile提供的方法，使多台电脑的时间差异小于1秒。
   6、Patient Demographics Query (PDQ)：患者基本信息查询――多个分布式应用程序可以使用某种特定的查询语法，向一个中心患者信息服务器查询患者信息，查询结果可以直接被应用程序所使用，包括患者的人口学基本信息，也可以包含就诊相关信息。
   7、Audit Trail and Node Authentication (ATNA)：审核所需记录与节点验证――描述了一个基本安全的节点所应具备的特征。1)，描述了安全节点所处的安全环境，包括用户标识、授权与验证、访问控制等。以便安全评审者可以判断环境是否满足安全要求。2)，定义了基本的安全审核要求。3)，定义了关于节点之间使用TLS或类似方法进行通讯时的基本的安全要求。4)，描述了在节点和收集审核信息的存储节点之间传输“审核消息”的架构。
   8、Personnel White Pages (PWP)：个人白页――访问获取机构内员工的基本信息。
   9、Cross-Enterprise Document Sharing (XDS)：跨机构文档共享――在属于同一个临床相关域内的多个医疗机构之间共享临床记录。此profile基于ebXml Registry标准、SOAP协议，HTTP协议和SMTP标准。此profile详细的描述了如何配置ebXml登记处，来支持跨机构的文档共享。
   
至2007年，IHE技术框架推出的集成事务图integration profiles有：
   影像医学Radiology：18个
   IT基础设施ITI：17个
   心血管学Cardiology：7个
   检验学Laboratory：6个
   病人监护协作Patient Care Coordination：5个
   眼科学Eye Care：3个
   放疗Radiology Oncology：1个
   病人监护设备Patient Care Device：1个

另外还将在药房、病理、质量、内镜、兽医学等方面，推出新的事务图profiles。

Transaction 译成交易有点生硬了,其实不如译成交互,或干脆不译.
Profile更多的译成(集成)模式.Actor,大家都知道它是做什么的,也可以不译.
对于这些词,圈内人只消理解即可,无需像romantic叫成"罗曼谛克"那样翻来翻去.
